Suivi des patients par leur médecin traitant après leur passage aux urgences d’un hôpital en territoire rural à Saint-Palais (64)

Abstract : Introduction: the Primary Care Physician (PCP) has to be the patient’s main referral. Unfortunately, in the emergency context, it’s rarely the case. The goal was to understand the factors that we could leverage in order to improve the patient’s management by the PCP. Aim: to identify the main determinants of the patient’s visit to their PCP shortly after a visit at the emergency department (ED). Method: from May 1 to May 30 2017, we conducted a prospective, observational study at Saint Palais on outpatients visiting the ED. Results: 102 patients were included after the ED visit. 74.5% were walk-in patients, mostly for trauma complaints. 30% have visited their PCP in the next 10 days following their visit. The most frequent chief complaints were to not being relieved (36%) and to inform the PCP (28%). Some patients had a follow-up visit with a specialist, and therefore didn’t see their PCP (39%). There was a significant difference in the follow-up visit for elderly patients (p<0.05), patients consulting their PCP at home (p<0.01), patients who benefit from the universal health coverage system (p<0.01), patients were referred to the ED by their PCP (p<0.01) and those who received the order to follow-up with the PCP by the emergency physician (p<0.01). Conclusion: insisting on patient’s follow-up by their own PCP after an ED consulting may improve the central position of the PCP in the management of patients.
