Transarterial chemoembolization (TACE) with irinotecan-loaded drug-eluting beads (DEBIRI) in refractory liver colorectal metastases (CR-LM)

Résumé

Introduction: define oncological effectiveness, OS, toxicity and adverse events (AE) of TACE using DEBIRI® in patients with unreseacable refractory CR-LM. Material and Methods: from September 2012 to November 2014, we included all patients presenting chemo-refractory CRLM from two Universitary Hospitals. Clinical and biological datas were collected. Minor and major AE and toxicity were evaluated according to the WHO classification. We used RECIST criterias for tumor response evaluation. OS and PFS survival were analysed. Results: 36 patients (25 men, 70%), mean age at 66±10 were included. Patients presented tumor lesion number of 1, 2 and >3 in 8%, 28% and 64%. PS was >1 in 22 (61%) patients. CRLM were localised in the right hepatic lobe in 53%, in both hepatic lobes in 36%, disease was synchronous in 67%. 17 (47%) patients had extra-hepatic disease. Patients received 1, 2, 3, 4, 5 cures, respectively in 14%, 36%, 36%, 11%, 3% cases. 47% went under the treatment initialy planned. Significant toxicity was observed in 29 (81%) patients: 29 (70%) pain (level III-WHO pain killers in 12 (33%)), 13 (36%) vomiting. Six patients were lost of sight. Tumoral response was efficient in 7 (23%, 3 complete responses), stable diseased and progressive were respectively in 11 (36%) and 12 (40%) after first evaluation. Mean follow-up was 6 months: PD was observed in 22 (61%) patients and death in 12 (33%A), corresponding to an OS of 16±4 and PFS of 2±0.4 months. Conclusion: tACE with DEBIRI in CRLM is associated with significant toxicity, but this therapeutic alternative leads to oncological control in 2/3 patients and acceptable OS and PFS according to the severity of the initial disease.
But : analyser toxicité et résultats oncologiques de l’injection IAH de DEBIRI® dans le traitement des MH-CCR réfractaires à la chimiothérapie. Méthode : étude rétrospective bicentrique, incluant tout patient porteur de MH-CCR réfractaires et traité par DEBIRI® de sept. 2012 à nov. 2014. Les données clinico-biologiques et de suivi ont été recueillies. Complications mineures, majeures et toxicité ont été relevées (critères OMS). La réponse tumorale a été évaluée selon les critères RECIST. La survie (SG) globale et sans progression (SSP) ont été calculées. Résultats : 36 patients (25 H, 70%) âge moyen 66±10 ans, étaient analysés. Les patients avaient 1, 2 et ≥ 3 lésions dans 8%, 28% et 64% des cas. Le PS était ≥ 1 chez 22 patients (61%). Les lésions étaient dans le foie droit (53%), ou bilobaires (36%), synchrones chez 24 patients (67%). 17 patients (47%) avaient une maladie extra-hépatique. Les patients ont reçu 1, 2, 3, 4 et 5 séances dans 14%, 36%, 36%, 11% et 3% des cas. 17 (47%) ont complété le traitement prévu. Au total, 29 patients (81%) ont eu une toxicité significative ou plus: 25 (70%) des douleurs (grade III, 12 [33%]), 13 (36%) des vomissements. Les données manquaient chez 6 patients. Parmi les 30 analysés, 7 (23%) avaient une réponse (3 réponses complètes), 11 (36%) étaient stables, et 12 (40%) progressaient à M1 (taux de contrôle initial de 60%). À 6 mois, 22 ont progressé (61%), 12 sont morts (33%). Les SG et SSP médianes étaient de 16±4 mois et 2±0,4 mois. Conclusion : l’injection IAH de DEBIRI® présente une toxicité notable, mais permet un contrôle tumoral dans 2/3 des cas, et un taux de survie acceptable étant donnée la gravité initiale de la maladie.
