La Lèpre à Mayotte : étude sur les nouveaux cas de 2006 à 2015

Abstract : Introduction: Mayotte is an overseas French department located in the heart of the Comoros archipelago which is a strong leprosy endemic zone. Leprosy or Hansen's disease is an infection caused by Mycobacterium leprae, which affects the skin and peripheral nerves. This disease is subjected to a strong migratory pressure so what is its situation in Mayotte in 2015? Materials and methods: it is a retrospective epidemiological study including all new leprosy cases in Mayotte between 2006 and 2015. We calculated prevalence and detection rates and performed statistical analysis according to the histological type and the age. Results: 475 new cases were diagnosed, 212 PB and 246 MB. The prevalence in 2015 was 3.11 / 10,000 inhabitants. Overall and PB’s detection rates are decreasing, MB’s is stable. The majority of patients diagnosed were of Comorian origin (63.0%). The patients under 15 years of age were mostly of French nationality. Discussion: efforts made in the management of leprosy control have led to a decrease in the prevalence. However, the detection rates curves show the endemic character of this region. The majority of patients of Comorian origin associated with a majority of children of French nationality proves the existence of two distinct outbreaks, one Mahorais, the other Comorian. Only better screening, better training of health professionals and, above all, close cooperation with the Union of the Comoros will allow progress towards the eradication of this disease. Conclusion: Mayotte and the Union of the Comoros remain an endemic zone of leprosy in 2015. Despite major advances in the fight against leprosy, there is still more progress to be made.
