Manifestations neurologiques et chikungunya : étude descriptive des patients adultes avec manifestations neurologiques hospitalisés au Centre Hospitalier de Polynésie Française de Tahiti au cours de l’épidémie de chikungunya 2014-2015 en Polynésie Française

Abstract : We report 64 cases of neurological events during the acute phase and/or during the 28 days following a chikungunya virus outbreak in French Polynesia that occurred from October 2014 to the end of March 2015. These symptoms were the cause of admission in our yard, mainly via the emergency room. 56 specific RT-PCR and 11 specific IgM were positive in the blood, 20 RT-PCR and 11 specific IgM were positive in the CSF, distributed in 41 central affections, 10 peripheral affections (Guillain-Barre) and 13 epilepsy relapses. 57.8 % of patients had a neurological history. 30 % of patients showed higher functions disorders, 26.5 % had locomotor disorders, 23.4 % had epilepsies, 6.25 % had unusual headaches, and 9% suffered malaise and/or falls. One death occurred during hospitalization. The standard biology did not find any peculiar characteristic of chikungunya. The cytochemical composition of CSF was sometimes discretely inflammatory. CT or MRI, when realized, showed no recent abnormality. Seven EEG were suggestive of encephalitis and eleven of a comitial crisis. The outcome of the neurological symptoms was generally good in a few days. The central and peripheral nervous systems affections during chikungunya outbreak are not that unusual, but the evolution is often good. The presence in our study of blood RT-PCRs, but especially of the positive RT-PCRs in the CSF, is an important argument for explaining these neurological affections by this virus.
