Place de l’anatomopathologie comme facteur prédictif de sévérité dans la papillomatose respiratoire récurrente juvénile. Vers un registre national de patients

Abstract : Introduction: Juvenile Onset Recurrent Respiratory Papillomatosis (JORRP) is a benign HPV-related condition whose evolution is unpredictable. Its management remains challenging for ENT surgeons despite the improvement of surgical techniques. Factors of clinical severity are: a low age at onset, an extra laryngeal extension and a great number of endoscopies. The purpose of this study was to assess wether pathology is a predictive factor of clinical severity of JORRP in our series of patients. Methods: data regarding clinical severity were recorded retrospectively in Spring 2017. After histological examination of patients’ biospies, we performed p16 and p63 immunostaining and low risk and high risk HPV chromogenic in situ hybridization (CISH). Correlation between clinical factors of severity and pathology was assesed with Pearson and Student’s tests. Results: thirty patients were included in this study. Mean age at diagnosis was 41,5 months-old. The average number of surgical procedures per year during patients’ follow up in our center was 2,8 per patient. Derkay-Wiatrak score was at an average of 15,9. Twenty cases of dysplasia have been reported in this series. p16 staining was negative in all but one patient. p63 staining was positive in all patients. Tests showed no correlation between clinical and pathology factors of severity. Conclusion: pathology was not a pronostic factor of clinical severity of JORRP in our series. Identification of accurate items in this study will lead to the creation of a local registry of patients presenting with JORRP in our hospital.
