Évaluation des motifs de consultations du service d’accueil des urgences adultes du Centre Hospitalier Universitaire de Pointe-à-Pitre

Abstract : Introduction. In France, the use of Emergency Room (E.R) is increasing exponentially (18 millions passages in 2013 against 7 millions in 1990). This study is motivated by this observation and as a matter of fact, seeks to assess ER user’s profile and the rate of unjustified consultations from the CIMU sort scale. Material and method. We conducted a prospective study using a hetero questionnaire, including patients who were 15 years old or older and who consulted at Point à Pitre University Hospital’s E.R between 8 a.m. and 11 p.m, from March 20th to March 22nd, 2017, and from March 24 to March 25 2017. It is important to note that patients referred by the SMUR or who did require initial management in the resuscitation area were excluded from this study. Results. As whole, this study was carried out on 322 patients. The user’s average age was about 51,7 (both genders were equally represented). The main reason advanced by patients for consulting was pain (17,3%) and 19,9% of the population was hospitalized. Overall, the two main reasons for using ER were the quick patients management and the probability of performing additional tests. 58% of the patients were CIMU 4 or 5, and among the CIMU 5 group, 55,8% consulted without a prior medical opinion. Additionally, 86% of these patients had registered general practioner and 71% did not know they could access medical centers on call. Conclusion. This study has found similar results to those achieved in Metropolitan France. It has highlighted a high proportion of unjustified medical consultations among patients attending ER without any prior medical advice. Therefore, awareness of the need for medical advice prior to ER visits and improved awareness of the existence of medical centers on call would likely be effective ways to reduce unnecessary medical consultations.
