Troisièmes molaires mandibulaires : évaluation prospective des pratiques professionnelles lors de la prescription Cone Beam Computed Tomography (CBCT)

Abstract : The extraction of mandibular third molars (3M) exposes the patients to a risk of injury the inferior alveolar nerve (IAN), hence there is the need for a preoperative radiographic examination. In some cases it could be necessary to prescribe a Cone Beam Computed Tomography (CBCT) which specifies the relation between IAN and 3M. A prospective study was carried out from december 2016 to september 2017 in the odontological service of La Timone hospital in Marseille, France. The aim of this study was to evaluate the professional practises for CBCT prescriptions before 3M extraction, and to compare the results of the observed anatomical relations for each imaging modalities. Study inclued 39 patients which represent 56 third molars. All CBCT was prescribed because there were a close relation between IAN and 3M on orthopantomogram (OPT) : contact in 28% of prescriptions and projection in 72%. When there was a contact, 46.2% of these cases were in contact on CBCT, from which pinched IAN were only 33.3%. When there was a projection, 81.8% of cases were in contact on CBCT, from which pinched IAN were 71.4%. Among 18.2% that were not in contact on CBCT, all had vestibular and round shape IAN. CBCT seems essential when projection of IAN is present on OPT. About radioprotection, all the exams seems justified, however if there is a contact it’s prescription seems not to be an obligation.
