Place du médecin généraliste dans la prise en charge et le suivi des patients sous anticancéreux oraux de type inhibiteur de tyrosine kinase : enquête d’opinion auprès des médecins généralistes à La Réunion

Abstract : Introduction: Oncology has undergone many evolutions due to an increasing number of patients, which required a new definition of the core role of the General Practitioner, but also due to new therapeutic advances such as oral focused therapies. With the current shortage of physician specialists, we focused our study on the opinion of General Practitioners from Reunion Island and their position in the treatment and follow-up of patients under tyrosine kinase inhibitor. Then, we analysed their justifications and expectations. Methods: A mixed sequential study was carried out. A qualitative part of the study included General Practitioners whose patients received a tyrosine kinase inhibitor treatment and were followed-up at the Saint-Denis CHU, in the hemato-oncology department, between November and December 2016. Then, a questionnaire was sent to liberal General Practitioner from Reunion Island. Results: Among 7 interviewed General Practitioners, their role was often described as a second role, staying away from the “scene”, position which was confirmed by the 42 physicians who answered the questionnaire and reported a low implication (39%, n= 16, IC [24,1 % ; 53,9 %]) or no implication at all (29,3 %, n= 12, IC [15,4 % ; 43,2 %]). A lack of information and education on existing treatments, and a lack of communication were respectively reported and confirmed by 61,9 % and 41,3 % of the physicians who answered the questionnaire. Yet, 87,8 % of physicians expressed a need for educative sessions. Conclusion: Although a core role is attributed to the General Practitioner in Oncology, the General Practitioner reports being excluded to the therapeutic treatment path of patients under tyrosine kinase inhibitor, often for lack of education and communication.
