La médecine évènementielle. Quelle est la représentation, à partir des freins et motivations, des internes concernant la médicalisation de manifestations ?

Abstract : INTRODUCTION : Reunion Island is a dynamic destination providing numerous religious celebrations, cultural and sports events and recreational shows each year that require important needs in medicalization. The organizers meet difficulties in providing the medical posts and are obliged to ask interns to ensure the health coverage of their entertainments. This study aimed at analyzing the barriers and motivations of the interns in providing medicalization in these public demonstrations as well as their expectations. METHOD : Qualitative survey using semi-structured individual interviews led with the interns of general medicine of Indian Ocean. The data analysis applied the principle of the anchored theorization. RESULTS : 14 interns were interviewed between November, 2017 and April, 2018. The interns were interested on this practice of the medicalization and we noticed an evolution on their barriers and motivations in providing medical care depending on their skills on this field. Regarding the unskilled ones, their barriers all converged on the feeling of incapability. As for the interns who already practiced medicalization, the risk assessment seems to be the main ground for refusal of a health coverage. Likewise, the interns were initially motivated by the financial attractiveness. Afterwards, they will choose to pursue the experiment of the medicalization by interest, whether for professional purpose, for financial reason or linked to the event itself. The awareness of the skills and professionalism required by a health coverage of a demonstration brings them to express expectations in terms of eligibility requirements but also training. DISCUSSION : Health coverages ensured by interns could be a good solution to the problem faced by the organizers. However, legal conditions and recommendations must be complied with (replacement license and/or holder of the thesis and/or skills in oxyology). In response to the expectations expressed by the interns, on-the-spot theoretical and practical training would allow them to be familiar with the regulations, to develop their skills in this field and therefore to consider requests for medicalization wisely.
