Impact d'un pharmacien clinicien au sein d'une équipe mobile d'évaluation gériatrique : axe prioritaire en chirurgie

Abstract : In accordance with RETEX Decree of 6 April 2011, elderly are considered at risk of Adverse Drug Reactions (ADR). Medication Reconciliation (MR) can help to reduce medicine-related illness substantially. During each patient evaluation, the Geriatric Assessment Outreach Teams (GAOT) carries out an up-to-date assessment of treatment. The aim of our study was to assess the impact of a pharmacist involvement in a GAOT, in order to optimize the therapy up-to-date assessment, to control treatments and to consider potential adaptations. Multi-medicated patients over 74 years-old, hospitalized in surgery or with an indication of ADR were included. For each patient, the pharmacist processed to a MR, ensures the quality of the pharmaceutical control and the pertinence review that were compared to the geriatric assessment report. All in all 66 patients were included. After the therapeutic assessment, 73 divergences were identified, including 41 that were not identified by the GAOT (20 with potential adverse outcome relevant). After the pharmaceutical control, 70 interventions were performed, including 53 by the pharmacist alone. After the pertinence review, 298 points were noted by the pharmacist, including 200 mutually noted with the geriatrician and 72 were unidentified and considered relevant by him. Pharmacist intervention in the GAOT, would allow a better optimization of treatments assessment and to provide pertinent and complementary proposals for therapeutic adaptations. This study allows a multidisciplinary approach thanks to the collaboration between departments, GAOT and the pharmacy to provide treatments and reduce medicine-related illness for patient benefit.
