Représentations et gestion des traitements par opioïdes forts ; vision croisée des patients et des infirmiers : les infirmiers

Abstract : Introduction :
Strong opioids suffer from a controversial image between major and narcotic analgesics. The objective of this study was to understand the representations and their consequences on the management of strong opioids for nurses.
Method :
A qualitative study of the semi-directed interviews is under way until data saturation, with the nurses of the Var and Bouches-du-Rhône, recruited by intentional theoretical sampling. The topics covered by the maintenance guide were the names, knowledge, management of strong opioid treatment on a daily basis, and the relationship of nurses to physicians and patients.
Results :
17 nurses with various profiles were recruited between September 21, 2017 and March 23, 2018. The nurses described a change in mentalities encouraged, and a relative confidence in the management of strong opioids. The main representation of opioids was that of an effective analgesic. However, their activities in practice demonstrate the presence of people still related to the administration of opioid treatment to patients. The main barriers expressed were related to fear of adverse effects, addiction, death and end of life.
Discussion :
The triangulation of data with the study of Céline Barres and the literature, allows us to see that beliefs about persistent strong opioids among actors: patients, nurses and doctors. It is therefore necessary to revalue the image of the strong opponent via information and training of all these actors, mainly targeting these received ideas. The practical training of carers seems an important lever, if it is well framed. There is a need to upgrade the doctor-nurse relationship to make therapeutic education more effective and reduce patient disruption.
