Efficience du dispositif médecin correspondant du SAMU dans la réponse à l’aide médicale urgente

Abstract : The “MCS” are general practitioners specially equipped and trained in emergency care.They work in isolated sectors.The regional health plan develops this organization to "ensure the entire population access in less than 30 minutes to emergency response”. The aim of this study is to observe the efficiency of this organization.This is a retrospective descriptive study, on the emergency response (ER) of the Haute Savoie MCS,for the year 2017. The ER were compared to MCS availability. The data comes from the SAMU74 and the «mountain doctor» association. The study concerns 20 MCS in 9 sectors. 2307 time slots have been filled,83% in season.434 AMU interventions took place, 78% in season, 186 carried out by the MCS, in 55% the relay by emergency unit of SAMU was canceled by the MCS. In "Seasonal" stations,MCS have a continuous presence. The response to emergency is 92% the day,60% the night. In the areas where the MCS work in medical groups, the seasonal presence is 83% the day, 73% at night, < 25% out of season. They respond to emergency in 69%.In areas where MCS operate alone, the seasonal presence is 61% the day,19% at night, < 10% out of season. They answers for half of the emergency in season, but take part in ¼ ER. All physicians seem to be equally involved. They intervene outside with equal shares whether alone or in groups. There is a strong correlation between the availability of MCS and their ability to respond to emergency. The device appears efficient in "seasonal" stations where all doctors are MCS;it is good in the stations where the MCS exercise in group;its efficiency is lower in sectors where MCS is alone,but is not inefficient.Development should focus on increasing MCS enrollment and group exercise ; with an assessment for each sector of the appropriate number of MCS and the period to be covered.
