Connaissance des internes de médecine générale français sur les directives anticipées

Abstract : Introduction: The main objective of our study was to make an inventory of the interns’ knowledge of French general medicine regarding advance healthcare directives. Method: This is an epidemiological, prospective, descriptive and multicentric study based on the use of an electronic questionnaire intended for French general medicine interns. Data collection took place from February 1st to May 15th 2018. Results: We collected answers from 543 interns with an answer rate of 6%. 99.26% of general medical residents said they knew the term "advance directives". They rated their knowledge of this device slightly above average with a score of 6.02 out of 10. Several knowledge points were assessed. Were known by the interns of general medicine: the date of creation of the advance directives for 24,67%, the majority of the criteria of validity of the device for less than 1%, their indications of consultation for 56,59%, their duration of validity for 50,46%, their revisable and revocable characters for 78,29%, their place of conservation for 37,48%, their place in the medical decisions for 84,53%, their binding for 64,64%. 54.17% of the interns indicated that they had encountered advance healthcare directives during their training and / or during their personal life. Almost all of the respondents thought that the device could be an aid to the care of their patients and that it was necessary to inform them. 97.38% thought that this would be one of their roles once graduated and installed. Conclusion: The teaching on the advance healthcare directives but above all on the management of the end of life and on the means of communication must be generalized and homogenized to the students of medicine and in particular in general medicine for a better care of the patients. A consultation scheme could help address the difficult topic of end-of-life counselling, and appropriate remuneration could help to forget the time-consuming effect of these discussions. By improving these different points, it would be possible to develop the drafting of advance directives and thus to find them a real utility.
