Parcours clinique de la femme enceinte consultant aux urgences générales ou spécialisées

Abstract : Introduction: Emergency attendance is increasing everywhere in France. So is the case in Marseille, at La Timone's Emergency Department (ED), which operates regularly in just-in-time situations. The consequences of such an activity are numerous, in a current context of gynaecologist shortage. The main objective of our study is to establish a map of the consultations of pregnant women in the general and specialized ED in Downtown-Marseille. Material and method: Our study is observational, retrospective and monocentric. The files of pregnant women at the ED of La Timone Hospital over the period from 01/01/2017 to 01/01/2018 are analysed, as well as those of patients transferred to the gynaecological ED of La Conception Hospital. Results: 103 pregnant women are included, 43 being aware of their condition, mainly in the first trimester and 60 discovering their pregnancy. Their average age is 28.5 years old, the 18-24 age group is the most highly represented. They usually came on their own and 91.3% of patients are not referred to us by their doctor. Consultation patterns are in decreasing order: urologic and digestive, cardiovascular, gynaecological. Biological samples are frequent, unlike ultrasound scans performed by the emergency physician. A specialist is asked for in only 26.2% of cases. A total of 59 patients go back to their homes straight away. 42 are transferred to the gynaecology department of La Conception Hospital, among which 23 go back home, with, for 12 of them, a new appointment to gynaecological ED. Three patients are admitted. Conclusion: This study reflects only a few consultation cases at the ED. However, at the ED, improvements could be achieved directly with our gynaecology colleagues. The general practitioner is one of the major contributors to the pregnancy follow-up, which can limit or enhance the use of ED by pregnant women. Subsequent studies that would examine factors statistically related to inadequate consultations at the ED might be relevant.
