Impact des antécédents psychiatriques sur la survenue d’un état de stress post-traumatique dans le post-partum : une revue qualitative et non exhaustive de la littérature

Abstract : Background: post-partum posttraumatic stress disorder (PTSD-PP) has reached growing attention in the study of its prevalence and risk factors. Diathesis-stress models have been suggested and the role of a psychiatric history is controversially described and seems to be underestimated. This study aimed to examine the impact of a psychiatric history on the PTSD-PP, and eventually, on other risk factors. Method: a non exhaustive qualitative review was carried out using the Pubmed and Psycinfo databases. We included 30 studies published between 1997 and 2017, examining the PTSD-PP and looking for a prior psychiatric history. Results were discussed according to their methodological issues. Results: we found a major direct and indirect impact of a psychiatric history on the reactivation, the onset and the maintenance of PTSD-PP symptoms. We discussed the hypothesis of adverse mechanisms following the type of past psychiatric disorder (PTSD or depression).We also found PTSD-PP due to obstetrical complications and it was independent of the existence of a psychiatric history. Methodological weaknesses have been discussed. Conclusion: having a psychiatric history can have direct and indirect impact on the existence of a PTSD-DD justifying specific attention during the perinatal period. More prospective longitudinal studies involving early pregnancy are needed to precise their mechanisms.
