Impact des interruptions de tâche sur la charge mentale de travail du médecin urgentiste

Abstract : Introduction. An interruption is defined as a response to an external stimulus (prompt) which causes the participant to stop the task they are engaged in, and switch to the secondary task. For doctors, their known impacts are : more prescribing errors, and more time to perform a task. Their consequences on workload (combination of cognitive and emotional costs to perform a task) have never been studied in emergency room. Methods. We performed a prospective, direct, observational study in emergency room, from Avril 23rd to June 29th, 2018. NASA-TLX (workload score) is collected before and after an observation session. An observer collects all interruptions. Final NASA-TLX is studied according to initial score, interruptions, and patients into health zone, by univariate and multivariate analyses. Results. We performed 33 sessions, bringing out 7 interruptions per hour. By univariate analyse, Final NASA-TLX score is significantly associated with interruptions (+5,5 points for every additional interruption, p=0,003), and with inital NASA-TLX (+0,63 points for every initial additional point, p<0,001). Discussion. With these results, we need other studies to see which interruptions are most disruptive. Conclusion. Impact of interrupting tasks on emergency doctor’s workload highlight the importance of discuss about emergency room organization, to avoid emergency doctor’s fatigue.
