Évaluation de la pertinence du scanner corps entier dans la prise en charge des traumatisés graves stables admis aux urgences du CHU de Caen : une étude observationnelle

Abstract : Introduction: severe trauma patient (TP) is a patient who has suffered trauma with high lesional potential. The severity assessment is defined in France by the Vittel criteria. When theses patients are stabilized, they must have a whole-body CT scans (WBCTS) according to expert recommendations. We studied the relevance of WBCTS in stable TP. Material and method: this was a monocentric, retrospective observational analysis. All patients over 18 years of age old, admitted to the emergency department, who have had a WBCTS at the university hospital of Caen were included from January Ith to December 31th 2017. Results: after selection, 235 stable TP were included. At least one SCE lesion was found in 46% of cases. The Vittel criteria were present in 82.6% of patients, "global assessment" accounted for 70% of the criteria. Patients who where over 50 years old and patients who have fallen were predictive of WBCTS lesions (p<0.0002). The time between admission and realization of the WBCTS was on average 120 minutes (± 87 min). The time to prescribe and perform the WBCTS was shorter for severe screening patients (p=O.001). Conclusion: WBCTS seems relevant in the management of stable TP at the university center of Caen, only 10% of patients had no Vittel criteria. The time of realization of the scanner remains to be improved. Work on triage in the emergency department could help to reduce this delay.
