Analyse de la valeur pronostique de l’expression immunohistochimique de VIPR2 dans une série rétrospective de 59 léïomyosarcomes utérins

Abstract : Introduction: uterine Leiomyosarcoma (ULMS) is a rare, aggressive tumor with limited therapeutic options. Except the stage, various clinico-pathological and molecular parameters showed very inconsistent prognostic values according to several studies. The intestinal vasoactive peptide receptor 2 (VIPR2), which is expressed in the myometrium and in leiomyomas, may be related to the prognosis of ULMS. The aim of this retrospective study is to examine the prognostic value of the VIPR2 protein expression by immunohistochemistry (IHC) and to compare it with other known factors. Material and methods: the following parameters were collected for 59 ULMS: stage, necrosis, mitotic index (MI), tumor size (TT) and cellular atypia. The immunohistochemical examination of VIPR2, p16, TP53, estrogen receptors (ER) and progesterone (RP) were performed on the corresponding Tissue Micro-Array blocks. The expression of VIPR2 was studied with 2 immuno histochemical scores (H-score, score-S). Results: IHC VIPR2 was positive for 45.8% to 49.2% of 59 ULMS. In univariate survival analysis, VIPR2 expression was significantly related to a shorter overall survival (OS) and no recurrence survival (PFS), regardless of the score used. With p-values for the two scores of P = 0.001-0.005 and P = 0.012-0.043, respectively. In Cox modeling, VIPR2 was shown to be an independent parameter in OS (P = 0.005-0, 007) with stage, RP, and TT. In PFS, only stage (P <0.0001) and MI (P = 0.0003) appeared as significantly independent. Conclusion: our study shows that in ULMS, the expression of VIPR2 is a pejorative independent prognostic factor in OS.
