Anomalies en video-capillaroscopie dans les dermatomyosites, le syndrome des anti-synthétases, les myosites de chevauchement et dans les myosites nécrosantes auto-immunes

Abstract : Objective: To compare nailfold videocapillaroscopy (NVC) changes from patients with dermatomyositis (DM), overlap myositis (OM), antisynthetase syndrome (ASS) and immune-mediated necrotizing myopathy (IMNM). Methods: We performed a prospective cross-sectional monocentric study. All patients with IMs over a 6-month period were analyzed by NVC for giant and ramified capillaries, tortuosities, capillary density, disorganization and, scleroderma pattern. Patients were classified as having DM, OM, ASS or IMNM for comparison. Clinical, biological and pathological characteristics were retrospectively recorded. Patients were further compared to a group of patients with systemic sclerosis (SSc). Results: We found that patients with IMNM and ASS present only with mild abnormalities on NVC compared to patients with DM and OM. OM patients have NVC pattern close to SSc. We could not determine NVC specific patterns associated to DM myositis specific antibodies. Conclusion: NVC is useful to distinguish between subgroups of IMs Patients. OM but not other IMs have a similar pattern as SSc.
