Colique néphrétique à La Réunion : l'exploration systématique a-t-elle sa place dans la prise en charge ?

Abstract : Context: kidney stone disease is a major public health issue. The prevalence is increasing in multiple countries, since second half of the past century. Reunion Island, a French overseas territory in the south-western of Indian Ocean is not spared by this condition. However, very few patients seem to have an etiologic exploration, for a variety of reasons. Objectif: the purpose of this study was to determine the place and the interest of an ambulatory exploration of the kidney stone disease, by a dedicated unit Method: this was a retrospective study. We included patients with renal colic after the July 1, 2017. We compared, on the basis of a phone questionnaire, explored and unexplored patients. Results: explored patients knew more often the causes of their own lithiasis (72% of explored patients versus 50% of unexplored patients; p=0,024). Explored patients more often adopted an adequate preventive strategy of the recurrence. In a second part, we show that kidney stone disease is very frequent in Reunion Island (we estimated the incidence is at 2 403 cases per million inhabitants). It differs from the one occurring in metropolitan France: environmental impact (dietary and climatic) seems to be exacerbated. The frequency of atypical lithiasis can suggest a genetic component. Conclusion: by allowing the means necessary to prevent lithiasis recurrences, a dedicated unit is beneficial to patients living in Reunion Island.
