Étude transversale à Marseille de la couverture vaccinale contre la grippe saisonnière 2016-2017 des enfants à risque et analyse des déterminants de l'intention vaccinale

Abstract : Introduction: In France, influenza vaccination is recommended for children with chronic disease. Objective: To establish influenza vaccination coverage for 2016-2017 in high-risk children consulting in a teaching hospital and to establish the conditions and determinants of influenza immunization. Methods: This is a cross-sectional, prospective, descriptive study. The included children were 6 months to 18 years of age. They consulted at the teaching hospital of Marseille and showed a disease that required the vaccine according to the Haut Conseil pour la Santé Publique. A survey to be filled by the parents, the children and then the pediatricians made it possible to specify their characteristics and their opinions on vaccination. Results: The analysis examined 402 children with chronic disease (26.6% of patients had respiratory disease, 24.4% immunosuppression, 24.1% neurological condition, 10.4% nephrologic condition, 8.2% Cardiac condition, 7.7% cystic fibrosis, 7.0% diabetes, 6.0% chronic liver disease, 2.5% obesity, 1.7% sickle cell disease and 5.0% had stayed in a post-hospitalisation clinic). Influenza vaccination coverage was 46.5% (95% CI [41.6; 51.4]) with a disease-dependent vaccination rate from 17.9% for diabetes to 93.5% for cystic fibrosis. The factors positively associated with vaccination were specialized follow-up, the recommendation of the vaccine by a physician, vaccination in previous years, receipt of the voucher, parental confidence in vaccines and some diseases: cystic fibrosis, nephrotic syndrome and transplantation. Conclusion: Influenza vaccination coverage in children with high-risk medical conditions consulting in a teaching hospital is insufficient and does not meet the target set by public health authorities.
