Revenir chez soi après une ou plusieurs hospitalisations : quels ajustements des parcours des individus par le recours à des services à domicile ?

Abstract : Home support services accompany elderly people over the course of their life as announced by the public authorities in their will to promote home aging. Even with those helpful services, individual and environmental elements can lead people to hospitalization. Going back home after hospitalization is a transition based on everyone’s experience while involving the elements that characterize their present. Disease trajectories and changes induced by hospitalization lead people to remodel their lifestyle at home. The use of home support services often happens at this stage. The service then accompanies people to find back balance in their life. Through two key sequences of people going back home, this work shows the adjustments made since leaving the hospital to continue a home lifestyle in autonomy.
