Amélioration d'un protocole d'analyse 3D bimanuel scénarisé du mouvement des membres supérieurs chez les enfants atteints de paralysie cérébrale spastique unilatérale

Abstract : Introduction: A new 3D bimanual protocol, “Be an Airplane Pilot” (BE-API), was created to evaluate upper limb (UL) motor impairments in children with unilateral Cerebral Palsy (uCP). Its feasibility and inter-trials reliability was excellent on 20 typically developing children (TDC) and 20 children with uCP. To improve the existing protocol, a new version, BE-API 2.0, was developed and its validation was evaluated. Materials and methods: Two new bimanual tasks were integrated. The angular waveforms, the maximum angles and the range of motion of shoulder, elbow and wrist joints, were calculated. 20 children with uCP (11.3±2.9 years) were matched on age with 20 TDC. To assess inter-session reliability, children with uCP performed two sessions separated from 2 to 4 weeks. Clinical measures including the Assisting Hand Assessment (AHA) and Abilhand-kids were passed by uCP children to explore relations with kinematic. Results: Among the first participants included, the feasibility of the BE-API 2.0 was excellent. In the two new tasks, degrees of freedom expected were mobilized in large range of motion: respectively shoulder elevation (X…), plan of elevation (X…), and wrist extension (X…). Children with uCP had lower shoulder elevation, plan of elevation, and wrist extension than TDC. Discussion: The new version of BE-API responds to the limits of the previous one, with reliable and discriminative tasks, which highlight movement impairments of uCP children in bimanual situation. Expending the final results of the study, the between-day reliability and comparison with TDC are very promising.
