Évaluation de la prise en charge des infections de prothèses totales de genou au CHU de Caen sur une période de dix ans

Abstract : Introduction: infection on total knee arthroplasty is a severe complication and the first cause of surgical revision after a total knee prosthesis. Several therapeutic options exist, depending on the type of infection. In this study, our objective was to evaluate the success rate of infections on total knee arthroplasty care, along with the knee function after infection management. Methods: this retrospective study included all patients operated for an established infection of total knee arthroplasty in Caen University Hospital over a 10 year period (2008-2017). Demographic and bacteriological data were registered in medical files. The primary study endpoint was the occurrence of therapeutic failure in infection care. An Oxford knee score was retrieved at the last check via mail or phone call. Results: we included 133 patients with a mean follow-up of 4 years. A 94.7% success rate was achieved for debridement of acute post-surgical infection and 75% for debridement of blood-borne acute infections. One-stage revisions were successful in 70% with a mean Oxford score of 22.5. We achieved a 71.7% success rate in two-stage revisions with a mean Oxford score of 23.4. Discussion: the results of infection care were satisfactory and consistent with literature data. Knee function results were poor and also reflected literature.
