Évaluation du bénéfice clinique et de la qualité de vie des patients implantés par des tiges de croissance électromagnétiques (TCE) par rapport à des tiges de croissance standard (TCS) dans le cadre de la prise en charge de scoliose à début précoce au CHU de Bordeaux

Abstract : Introduction: MCGR were developed to decrease the number of iterative surgical operations associated with treatment of EOS. Even if these treatments have largely been agreed upon, there is a lack of understanding of their impact on patients’ and families’ quality of life. The objective of the study is to compare quality of life of patients and their families in TGR and MCGR patients. Methods: This is a retrospective study based on a quality of life questionnaire and on electronic medical record of patients with EOS and implanted by MCGR or TGR between 2016 and 2019. Quality of life was assessed by a questionnaire composed of previously validated 24-item early onset scoliosis questionnaire (EOSQ-24) and 10 additional items that specifically assess quality of life around distractions. The questionnaire responses were analysed according to clinical data (curve correction, number of complications and of surgical procedure and of rods lengthening). Results: Fifteen children with EOS were enrolled, 7 TGR and 8 MCGR. Groups were similar in age and Cobb angle at time of surgical implantation. Age at the time of questionnaire and mean length of follow-up were significantly different; patients were older (15.1 vs. 11.9 yr) and had longer follow-up (5.9 vs. 1.9 yr) in TGR. Deformity correction and complication rates were similar between groups. Quality of life data revealed superior outcomes during distraction period in the MCGR group. However, at the time of questionnaire, overall satisfaction was not superior in MCGR than in TGR. Discussion: This indicates that the MCGR do not revolutionize patient satisfaction. The major advantages of TCE remain the low exposition to spinal surgeries, the reduction of infections and the more natural growth.
