Émergence des nouveaux produits de synthèse et protocole de l’étude SkiTox : consommation des nouveaux produits de synthèse et apparentés chez les patients intoxiqués pendant la saison de sports d’hiver

Abstract : Since 2000, the frequency of admission to the emergency department following acute intoxication with new psychoactive substances (NPS) has dramatically increased. These new drugs are synthesized by clandestine laboratories and then marketed via Internet. Their molecular structures are similar to classic drugs, with a much higher potency. These new psychoactive substances may have stimulating effects, others provoke hallucination and some are sedatives. Consumers buy the substance according to the desired effect. Synthetic cannabinoids and cathinones are the most common frequently encountered families of NPS on the market, usually consumed in a recreational setting. In addition to the usual medical test performed in the emergency department (electrocardiogram, ionogram and serum creatinine level, blood glucose, blood cell count, coagulation assessment), it is necessary to carry out samples for preservation purposes. Thus, these samples can be sent to a reference laboratory for the identification of NPS in case of atypical evolution of an intoxicated. The lack of knowledge of NPS and their power of action and their toxicity may be responsible of inadequate or suboptimal medical care. Medical complications must be looked for, and physicians should pay particular attention to the diagnostic and treatment of serotonin syndrome. Medical care is based on general symptomatic treatments such as intravenous hydration and the correction of hydroelectrolytic disorders, the management of hyperthermia, agitation, and disorders of consciousness. There is no antidote to NPS except the punctual use of naloxone and flumazenil depending on the supposed ingested substance.
