HE4, nouveau marqueur d'infertilité masculine ?

Abstract : Introduction : Infertility is a common reason for asking health professionals. In 20% of cases, only the man is involved. Some infertilities, labelled idiopathic, could be due to abnormalities in sperm molecular maturation. HE4 has been discovered in the distal part of the epididymis, an organ known to be involved in sperm maturation where spermatozoa acquire their mobility and their fertilizing abilities. HE4 assay using a simple, robust and not expensive method could be applied in routine clinical practice to explore male infertility. The main aim of this study was to determine whether patients with abnormal sperm parameters had different HE4 serum levels than men with normal seminograms.
Materials and methods : This is an observational case-control study. It compares a group of men with at least one sperm impairment, the "cases", to a "control" group of men with a normal seminogram paired on age. We included every man from 18 to 50 years-old consulting in Reproductive Medicine Unit of the University Hospital of Nice. HE4 serum level was determined with EIA assay distributed by Fujirebio Diagnostics, Gothenburg, Sweden.
Results : Between July 2017 and April 2018, we included 120 patients. The HE4 mean serum value of the "cases" was not different from that of the "control" group (respectively 45.87 +/- 22.71 pmol/l and 44.12 +/- 15.42 pmol/l, p = 0.6233). Nevertheless, there was a significant difference between the mean HE4 value in patients with less than 30% progressive mobility and those without asthenozoospermia (respectively 54.8 +/- 23.54 pmol/l and 42.94 +/- 15.87 pmol/l, p = 0.0441).
Conclusion : With our study we do not discover a new diagnostic tool to explore male infertility but it would be interesting to investigate the link between HE4 and asthenozoospermia.
