Facteurs associés à une mauvaise observance thérapeutique chez les patients vivant avec le VIH à l’hôpital Albert Schweitzer au Gabon en 2019

Abstract : Introduction : Non-compliance can have serious repercussions for the patient, his partners, the evolution of the epidemic and the country's health budget. The purpose of this study was to identify factors associated with poor adherence to treatment in patients living with HIV in Lambaréné, Gabon, in order to propose solutions to improve patient care. Method : In an analytical cross-sectional study that took place over 5 months at Albert Schweitzer Hospital in Lambaréné, compliance was assessed by a combination of two methods: medical interview and punctuality of appointments. Results : Among the 90 patients recruited, 80% were women, the average age was 42 years, and 66% were not observant. The main reasons for non-compliance were: misunderstanding of the disease (60%), denial of the disease (32%), financial accessibility (32%), poor medical care (30%). Older patients were more observant (95% CI, OR=1-1.1, p=0.044). The seniority of the disease is a factor of non-compliance when it tends to increase: between 1 and 5 years of seniority (CI 95%, OR=0.02-0.028, p=0.001), and more than 5 years of seniority (CI 95%, OR=0.03-0.37, p=0.001). Conclusion : Comprehensive and better quality medical care is needed to improve compliance, based on therapeutic education and psychosocial support.
