Détermination isotopique du débit de filtration glomérulaire par le pentétate de technétium en pédiatrie. Alternative au médicament radio pharmaceutique de référence, l'édétate de chrome

Abstract : Renal function can be assessed by many more or less precise and reproducible methods. Glomerular Filtration Rate (GFR) gives the true function of kidney since its value is directly correlated to functional nephrons quantity. In clinical practice, is recommended to use GFR estimation formulas derived from creatinine. However, values given by these formulas have many intra- and interindividual variations that can complicate the assessment of renal function. In some cases is necessary to have an accurate measurement of GFR. To do this, tracer used must respect several parameters: free glomerular filtration, absence of reabsorption and tubular secretion, absence of pharmacological action, toxicity, metabolism and plasma proteins binding. Isotopic measurement by chromium edetate is the reference method because this radiopharmaceutical drug (RPD) respects all these parameters. However, the supplier laboratory stopped its production line in December 2018. The objective of this work was to research and implement an isotopic measurement technique for GFR in nuclear medicine department of Toulouse University Hospital. The alternative of choice is technetium pentetate. It is an RPD that gathers all the parameters necessary to measure GFR. The technique for measuring GFR with technetium pentetate was validated and compared to reference method in two adults with normal renal function. Subsequently, a retrospective study was conducted in pediatric patients with renal function monitoring by comparing the isotopic measurement of GFR to that estimated by Schwartz formula. These studies allowed us to validate the GFR isotopic measurement technique using technetium pentetate.
