Étude rétrospective monocentrique pour décompression isolée versus arthrodèse sur un niveau chez des patients ayant un canal lombaire étroit équilibré

Abstract : Introduction: when it comes to patients with a symptomatic lumbar spinal stenosis, the surgical decision wasn’t clearly made between the simple decompression procedure and the decompression associated with transforaminal lumbar interbody fusion (TLIF). We introduced a new decision tool (ODHA) that was a global spinal parameter. All the patients concerned by our study had normal ODHA. The purpose of this study was to show that for a patient with a conserved sagittal balance and a normal ODHA, simple decompression compared to decompression associated with TLIF was sufficient for lumbar spinal stenosis surgery. Material and methods: the population of our single institution study was divided into two groups, a recalibration group (60 patients) and a TLIF group (50 patients). The included patients had a lumbar spinal stenosis on one level, they did not present spondylolisthesis and frontal deformity. The clinical (SF 36, ODI, EVA) and lordosis gain (L4S1 and L1S1) scores were compared preoperatively and retrospectively one year in both groups using the Student and Wilcoxon subgroup analyzes. Results: our results showed a significant one-year improvement in lordosis gain and clinical scores in both groups, but no significant difference between the two groups for improved clinical scores and lumbar lordosis gain. Conclusion: the purpose of this study was to establish preoperative planning in patients with symptomatic lumbar spinal stenosis on one level. We had shown that in case of global balance respected, the simple decompression gesture was preferred over a decompression associated with a TLIF.
