Description de la répartition des familles de situations cliniques rencontrées par les maîtres de stage universitaires de niveau 1 de l’UPEC dans leur activité de soins

Abstract : Introduction. The goal of this study is to describe the distribution of the activities of University teaching GPs (TGP) at the University Paris-Est-Créteil (UPEC) according to eleven families of clinical situations defined by a reference system of general medicine.
Method. A descriptive transversal study by data collection scoring grid data of the eleven families of situations, was conducted by observing their TGP’ consultations. during 3 consecutive weeks, on November-December 2017 and March 2018 Sociodemographic data concerning patients were also collected.
Results. This study led to a total amount of 2,025 consultations and coded 3,442 sub-families of clinical situations (1.70 families / consultation). Most encountered clinical situations were linked to chronic diseases (37.42%), acute problems (28.09%) with the sub-family non-urgent acute somatic complaints (21.03%). 4.45% of the cases were not coded. Age, socio-professional category and gender of the patients influenced the distribution of the family’s clinical situations.
Discussion. This study shows the great diversity of families of clinical situations that GPs encounter. A clinical situation exposition inadequacy seems to arise between the resident GPs and the teaching GPs. Such a discrepancy should incentivise to rethink and reflect on the ambulatory internship organization. The significant prevalence of non-indexed situations could lead to a family medicine situations benchmark review in order to broaden its spectrum.
Conclusion. UPEC teaching GPs encounter the eleven families medicine clinical situations in different proportions.
