Les comportements sexuels inappropriés chez la personne âgée démente : prévalence, impact sur les aidants, rôle du médecin traitant

Abstract : INTRODUCTION : Dementia is a public health problem. In addition to memory loss, behavioral and psychological symptoms of dementia are numerous and impair health and quality of life of the caregivers and the patients. Among these symptoms, inappropriate sexual behaviors (ISBs) are poorly studied compared to other BPSDs, although they cause great distress. The purpose of this study is to determine the prevalence of ISBs, their impact on the quality of life of caregivers and the place granted by the close relative to the referring physician that will help him manage this disorder.
METHOD : A quantitative, observational and retrospective study was conducted at day hospital Paul Brousse in Villejuif involving 130 monitored patients who were included consecutively between September 2016 and February 2017. A questionnaire was submitted by telephone to the main caregiver. Descriptive and comparative statistical analyzes were made.
RESULTS : We found an ISB in 18.46% of the patients. While 58.33% of the caregivers had not identified the disorder of their loved one as an ISB before answering the questionnaire, 100% said ISB had an impact on their daily lives. 86.15% of the respondents identified the general practitioner as their preferred contact to address this topic.
CONCLUSION : ISBs are a common manifestation of dementia. Given the significant burden on the caregiver, it seems necessary to enhance general practitioners’ awareness about this problem, so that the topic of sexuality is systematically addressed during medical consultations of elderly demented patients. To achieve this, specific assessment tools and a universal definition of CSIs should be developed.
