Description de la répartition des familles de situations cliniques rencontrées par les maîtres de stage universitaires de niveau 1 de l’UPEC dans leur activité de soins

Abstract : Introduction. A General Practice benchmark described eleven family medicine clinical situations. No study analysed GPs’ activities through these eleven indexed situations. The goal of the study consists in describing the allocation of these situations in Paris-Est Creteil (UPEC) University teaching GPs (MSU) day-to-day activity.
Methods. A cross family medicine clinical situation scoring grid data based study was carried out by UPEC resident GPs observing their teaching GPs’ (MSU) consultations. Data were collected on September 2017 on a 3-following week period basis. Patients and MSU’s sociodemographic data were collected as well as part of the study.
Results. The study led to a total amount of 1,466 observed consultations and 2,664 coded family medicine clinical situations (i.e. 1.82 family medicine clinical situations per medical consultation). Most encountered clinical situations were linked to chronic pathologies (34.38%), acute problems (25.34%), and then real or felt emergencies (10.89%). A significant proportion of unrecognised clinical situations (3.68%) was coded. The family medicine clinical situation distribution was directly linked to patients’ age, socio professional category and gender data.
Discussion. This study sheds lights on the family medicine clinical situations’ diversity encountered by the GPs in their daily activity. A clinical situation exposition inadequacy seems to arise between the resident GPs and the teaching GPs. Such a discrepancy should incentivise to rethink and reflect on the ambulatory internship organization. Non indexed situations could lead to a family medicine situations benchmark review in order to broaden its spectrum.
Conclusion. UPEC teaching GPs encounter the eleven family medicine clinical situations in different proportions.
