Apport de l’analyse quantitative du lobule clair dans le diagnostic tomodensitométrique de la pneumopathie d’hypersensibilité chronique fibrosante

Abstract : Purpose: to assess the diagnostic accuracy of decreased attenuation lobule extension quantitative analysis, within chronic interstitial pneumonias as a CT scan diagnostic tool of fibrotic chronic Hypersensitivity Pneumonitis (HP). To design a CT scan quantitative method of decreased attenuation lobule extension, analysing its reproducibility. To evaluate diagnostic accuracy of visual analysis for chronic HP and its reproducibility. Materials and methods: retrospective monocentric study comparing a group of patients followed for a chronic HP with a control group composed of patients with Idiopathic Pulmonary Fibrosis (IPF) and Non-Specific Interstitial Pneumonia (NSIP). Histological proof determined Patients’ inclusion. Visual analysis compared the usual semiological signs for interstitial disease and more particularly assessed the presence of decreased attenuation lobule. Semi-automatic quantitative analysis evaluated decreased attenuation lobule extension. For each analysis, a dual reading allowed reproducibility testing. Results: a group of fifteen patients with chronic HP was compared with a control group composed of seventeen patients with IPF/NSIP. The semi-automatic quantitative analysis enabled diagnosis of chronic HP with a sensibility and a specificity greater than 80% for decreased attenuation lobule extension threshold of 10%. Inter-reader reproducibility was excellent (ICC=0.89). Simultaneously, visual analysis brought a good sensibility (87%) of decreased attenuation lobule for chronic HP diagnosis with a poor specificity (41%). Conclusion: decreased attenuation lobule turned out to be both sensitive and specific for chronic HP diagnosis, exclusively through semi-automatic quantitative analysis.
