Incidence de la récidive suicidaire dans l’année qui suit un premier passage à l’acte : une cohorte historique avant la modification de l’offre de soins : étude RAPPAS

Abstract : Background: The antecedent of self-harm represents the major risk factor for suicidal recurrence, with an increased risk in the year following the acting out. The provision of care in the Grenoble area has recently diversified with the creation of the Intensive Ambulatory Therapy Center (CTAI). Evaluating the effectiveness of this device requires knowing beforehand the incidence of the first suicidal recurrence. Objective : To estimate the incidence of first suicidal recurrence the year following a first suicide attempt. Material and methods: This retrospective observational study focuses on a cohort of 154 first-suicide attempters admitted to the emergency department (ED) of the CHU in 2016. Patients with documented schizophrenia, bipolar disorder, or mental disability, as well as non-Francophone patients were excluded from the study. Results: 154 patients were included in our study. Twenty patients, or 12.9%, [IC95: 8-19] re-attempt within one year of their first self-harm. No suicide was recorded in our cohort. Clinical and socio-demographic characteristics were comparable among recidivists and non-recidivists. Half of the patients considered had recurred within 3,6 months after emergency room consultation for 1st self-harm. 41 patients (26%) were hospitalized in psychiatry within 10 days of their emergency department visit for a first suicidal act while 33 patients (21%) were readmitted to the emergency department at distance from their 1st suicide-attempt. The majority of these remote hospitalization from the first judicial crisis (68%) were related to mental health issues. Conclusion: The rate of suicidal recurrence in our cohort is comparable to the recidivism rates described in the literature.
