Les particularités de la prise en charge globale des personnes obèses : le point de vue des internes de médecine générale de Grenoble, une étude qualitative

Abstract : Introduction: The general practitioner is at the forefront of the global medical care of the obese patient. But he faces various difficulties, which can lead to discouragement. Ideally, the doctor should refrain from any judgment, but medical stigmatization of obese patients exists. What do interns of general medicine think ? Methodology: Qualitative study by semi-directed individual interviews, carried out on interns of general medicine from April to June 2019. Results: These 13 interviews revealed four main representations of the global medical care of the obese patient by the interns. These evoked an image of fragility and a need to take charge of obesity. There was an empathic vision, with a fear of hurting patients and the awareness of stereotypes. Sometimes a pejorative representation of obesity with medical, social and psychological stigmatization was revealed. Finally, incomprehension and resignation appeared. Discussion: The stigmatization of obesity is apparent in this study as in the literature. It can be explained by the influence of society on the interns of medicine, and by a divergence of perception between caregivers and patients. It has adverse effects on the patient. In addition, there is a focus on hyperphagia and physical inactivity. However, it is recognized that obesity is multifactorial. Conclusion: Interns of medicine have different visions of obesity. The same difficulties as their elders are found, and the desire to have a better approach to the obese patient. Training professionals and listening to patients in a therapeutic education approach seem to be areas for improvement.
