Recherche de facteurs prédictifs de réponse au traitement par Discogel intra-discal chez les patients lombalgiques et lomboradiculalgiques d'origine discale

Abstract : Introduction: low back pain and sciatica are a major public health problems. This has become a priority since the past decades. The Discogel, percutaneously administrated, is used for the treatment of lumbar sciatica or low back pain caused by a herniated disc. Studies report 25 to 30% of failure rates. It is routinely used since many years in the physical and rehabilitation medical service of the University Hospital of Bordeaux. Aim: the research of predictive factors of efficacy in the use of Discogel in sciatica or low back pain caused by a herniated disc. Method: retrospective descriptive study of the files of patients with sciatica or low back pain caused by herniated disc, treated by injection of Discogel between July 2014 and the 4th June 2018. Treatment’s efficacy is defined by the Minimum Clinically Important Improvement and Patient Acceptable Symptom State values applied at the Quebec Back Pain Disability Scale: a score below 40 after treatment and an improvement of that score equal to or higher than 20%. Results: 84 files was analyzed. Treatment’s efficacy criteria was found in 59 files. Finger-floor distance, which represent trunk flexibility, is predictive of Discogel’s efficacy in sciatica or low back pain caused by disc herniation. Sex, time of medical leave and the kind of disc herniation are not predictive of efficacy. Conclusion: the discovery of predictive factors of efficacy in the use of Discogel in sciatica or low back pain caused by a herniated disc might be used to decrease the failure rate.
