Évaluation de la prise en charge de la douleur aiguë chez l’adulte aux urgences du Centre Hospitalier de Périgueux : Efficacité de l’analgésie après la mise en place d’un protocole de prescriptions anticipées à l’accueil

Abstract : Study objective: pain is a topical issue in emergency departments, with 60% patients reporting pain. If analgesia must be optimal, many brakes persist and oligoanalgesia still remain in one third of patients at discharge. The main objective of our study is therefore to describe the effectiveness and quality of the management of acute pain after the implementation of a protocol of anticipated prescriptions in patients over 18, in order to compare the results obtained with those of the study carried out in 2018. Method: this is a retrospective monocentric observational study before after, performed at the Hospital Center of Périgueux (HCP).Inclusion criteria included admitted communicating adult patients with moderate-to-severe pain. The main outcome was defined by the effectiveness of emergency room pain relief represented by pain assessment at discharge. Analgesia was considered sufficient or of quality when the Numeric Rating Scale (NRS) was less than or equal to 3/10 at discharge. Results: 1048 patients were included from March 19, 2018 to June 16, 2019. There was a very slight increase in the rate of patient relief (62.6% 95% CI [58.4- 66.8] for period I and 64.9 % IC95% [60.6-69.2] for period II) but this difference is not significant. Conclusion: this study has shown a change in practices, without being translated into a significant improvement in the overall effectiveness of analgesia in the service. A new study would be needed remotely to evaluate with more hindsight the potential impact of this protocol.
