Premiers remplacements en médecine générale : intérêt du Stage Ambulatoire en Soins Primaires en Autonomie Supervisée (SASPAS) ; une étude qualitative auprès de remplaçants en médecine générale à La Réunion

Abstract : The training of interns in General Practice in Reunion is young. Besides, the substitute doctors practicing there are more and more numerous. The level 2 “Stage Ambulatoire en Soins Primaires en Autonomie Specialisée (SASPAS)”- or Ambulatory Course in Primary Care in Supervised Autonomy-, aims at better preparing these students who are finishing their curriculum. Objective: to determine the role of SASPAS in the preparation of the first replacements and to highlight other facilitating elements. Method: this was a qualitative, descriptive and comparative study, encompassing seven substitute physicians who went through SASPAS during their years as interns, and seven substitute physicians who did not attend the training. All of them were subjected to a similar questionnaire, then each interview was revised according to a model of inductive analysis, by manual coding. Results: SASPAS seems to be a factor facilitating the first replacements as general practitioners, by influencing mainly the ability to perform administrative and medico-administrative acts, and it is perceived as the training which is most in line with the future profession. It is also facilitating with regard to creating a medical network. Other facilitating factors have also emerged, primarily the Gynecology-Pediatrics combined course, but also the importance of the Level 1 practicum. Conclusion: in Réunion, the SASPAS is a training that seems to facilitate the first medical replacements. Improvements could be considered, such as the introduction of a "practical booklet for the first replacements" and an increase in the availability of certain trainings.
