Utilisation des antalgiques de palier 2 chez la personne âgée de plus de 80 ans : focus sur les pratiques chez les médecins généralistes du département du Sud Manche

Abstract : Introduction: low opioids are the most prescribed analgesics after paracetamol in the elderly. However, they are providers of undesirable effects that are not negligible in the elderly, often polypathological and polymedicated. The aim of the study was to evaluate low opioids prescribing practices in the patients over 80 years, among general practitioners in the Sud Manche area. Method: in this quantitative, observational, descriptive and cross-sectional study, liberal general practitioners from the Sud Manche were interviewed by online questionnaire. Results: the response rate was 23.63%. All general practioners surveyed used low opioids to treat pain in the elderly. None of them were prescribing strong opioids at the outset for pain resistant to non-opioid analgesics. 46.15% of general practitioners prescribed opium powder, 34.62% codeine and 19.23% tramadol. General practitioners were poorly trained on the subject of painkillers. One-third of them said they were having difficulty with this prescription. Discussion: a new expert consensus on analgesics has been held in 2017. The prescribing practices of low opioids by general practitioners appear to be generally in line with these recommendations, but there are still some differences . Low opioids are not easy to prescribe but are essential for the physician in the treatment of pain in the elderly. Increasing the training of general practitioners would help them to better prescribe them.
