État des lieux de la prise en charge des cancers du col utérin localement avancés : étude rétrospective multicentrique française sur une cohorte de 512 patientes (groupe de recherche FRANCOGYN)

Abstract : Regarding the management of locally advanced cervical cancer, current recommendations indicate that para aortic lymphadenectomy alone should be performed if there is no para aortic lymph node dissemination diagnosed by PET - CT. In the opposite case, a concurrent chemoradiotherapy without lymphadenectomy is indicated with extension of the radiation fields to the para aortic area. However, some parts of this therapeutic strategy remain debated. The aim of our study was to carry out an evaluation of French practices in referent centres. We retrospectively collected data from 512 patients with histologically proven cervical cancer, at an advanced FIGO stage (IB2 to IVA), managed between January 1999 and January 2015, in one of the nine centers of the FRANCOGYN research group. 36% of the patients did not have PET-CT and 18% had no pre-therapeutic nodal staging. Isolated pelvic lymphadenectomy, isolated para aortic lymphadenectomy, or both were performed for 13%, 38% and 47% of patients, respectively. 90% of patients had concurrent chemoradiotherapy, which was completed by total hysterectomy in 50% of cases. Some elements of the management of locally advanced cervical cancer are not consensual. Although concurrent chemoradiotherapy is the standard of care, there is a heterogeneity in the evaluation of lymph node status. In addition, performing a hysterectomy after concurrent chemoradiotherapy is also very heterogeneous and its place remains to be defined in the therapeutic strategy.
