Anorexie mentale de l’enfant et de l’adolescent en soins primaires : une enquête de pratique auprès des pédiatres et médecins généralistes de Gironde

Abstract : Background: the care pathway for patients with anorexia nervosa (AN) is long and com-plex with significant diagnostic and addressing delays. International recommendations support the earliest possible identification and specialist care. Aim: investigate current practices of identification, organization of care and exchanges with the child psychiatrist among primary care physicians (GPs and paediatricians), taking care of children and adolescents with AN, in Gironde. Method: descriptive transversal study, using an online questionnaire, validated by a panel of experts sent by email to 1625 doctors. Collection of data from november to june 2019. Results: identification in primary care suffers from lack of training of primary care practitioners. More than 90% of them would like to have access to a validated screening questionnaire for the paediatric population. Nearly 40% of practitioners involved in the follow-up of patients with AN report carrying and announcing the diagnosis, with no significant difference between them except for age. More than 9 out of 10 practitioners go to a child psychiatrist in a public hospital for about 70% of cases. More than 90% of the physicians surveyed do not receive the information from the medical file "in a timely manner", more than one practitioner out of three considers the quality "mediocre" and nearly 60% deplore an "insufficient" quantity. Conclusion: this is the first descriptive study questioning the practices of GPs and paediatricians as well as their exchanges with child psychiatrists about anorexia nervosa in children and adolescents. The weight of representations of AN among GPs and paediatricians and the link with the management of this disease in primary care remain to be explored. The development of care networks, training activities and the development of screening tools should guide future research on child and adolescent AN.
