Femmes victimes de violences sexuelles : étude qualitative sur les freins ressentis au recours aux médecins généralistes

Abstract : Introduction: 14.5 % of women are victims of sexual violences in France during their lifetime. These violences have short- and long-term consequences for the health of the victims. Yet, only 16 % of all victims of sexual violences in France consulted a doctor. Aim: to explore the barriers to using general practitioners (GP) experienced by women who are victims of sexual violences. Method: 23 semi-structured individual interviews were conducted with women survivors of sexual violences (rape, attempted rape ...) during this qualitative study. The thematic analysis of the verbatims was carried out by two researchers. Results: the results highlighted three major issues. The first concerned the difficulties in identifying oneself as a victim : amnesia of violence, difficulties in naming violence, lack of knowledge of their repercussions. Secondly, the barriers related to a "silence law", included the societal taboo, the family secret and threats from the agressor, which generated strong inhibitory thoughts (shame, guilt, pain, fear...). The third issue was specific to GP. The barriers were the perception of sexual violence as not falling under primary care, an unsatisfactory doctor-patient relationship, the absence of question about sexual violences, the limited time of the consultation. Finally, the thinking was completed by a theme about the negative reception of this word by the GP which undermined the continuation of care. Conclusion: the results of the study highlighted obstacles to the use of GP related to the patient-doctor relationship but also important psycho-social barriers. A specific public health policy to sexual violences seems essential to create a beneficial socio-cultural environment to the revelation of violences and the care of victims.
