Abord du sinus frontal‎ : bases anatomiques et systématisation des différentes voies d’abord chirurgicales‎

Abstract : In spite of technical advances and operating experience of surgeons, endoscopic approach of the frontal sinus remains one of the most complex surgery in rhinology. Because of complex and variable anatomy of the frontal sinus and narrowness drainage pathway, it is a real challenge for the rhinologic surgeon. The choice of indications and the achievement of surgery under good conditions limit any risk of meningeal and orbital breach. The development of mucosal flaps showed a decrease of secondary stenosis and a improvement of functional results. Even if it does not allow to get rid of a perfect anatomical knowledge, the development for a decade of sinusonavigation is an undeniable help in frontal sinus surgery. Gradually replaced by endoscopic approach, the external approach is nevertheless useful in some indications. Its realization and its learning must be continued. The recent emergence of so-called combined approach, and more recently of frontal sinus balloon sinusoplasty, demonstrates the multiple choices of management of frontal sinus pathology. Postoperative care are essential in frontal sinus management.
