Télémédecine et téléexpertise en plaies et cicatrisation : ressenti de médecins généralistes béarnais

Abstract : Introduction: at the time of deployment of telemedicine, teleconsultation and teleexpertise are about to change the practices of general practitioners (GPs). But what do they think of these new practices ? Our study aimed to evaluate the perception of GPs after using the teleexpertise in the management of patients wounds in nursing homes and home-hospitalization. Methods: we proceeded to a qualitative study by semi-directive individual interviews with open coding and an analyze by Grounded Theory Method: GPs interviewed were the referring GPs of patients who have benefited of teleexpertise management for their wounds. Results: 13 interviews were conducted. The telexpertise in wounds and cicatrization seems to be a validated practice by the GPs. Their lack of availability explains that they are ready to delegate these acts to trained caregivers, if they stay informed of the care provided. A lack of information has been reported by GPs, regarding both local projects and the law, which may explain their lack of involvement. An improvement of the network seems essential to them in order to promote telemedicine. If GPs are interested in this practice, they want it to remain complementary to traditional clinical medicine. Finally, used for a long time and daily, GPs would like the phone-based medical advice become a recognized and valued act. Conclusion: true revolution in their exercise and sometimes even in their conception of medicine, telemedicine requires information and training campaigns to allow GPs to appropriate this new tool.
