Conséquences des chirurgies orthognathiques sagittales sur les voies aériennes supérieures et le SAHOS : revue systématique de la littérature

Abstract : OSA syndrome was described in 1976 by Professor Guilleminault. Its prevalence remains low in the general population; the serious consequences that it can cause, engages the orthodontist to detect and address for an adapted care. Non-invasive therapies, such as CPAP or mandibular advancement orthosis, can prevent aero-pharyngeal collapse and reduce cardio-cerebrovascular risk. Orthognathic surgery (mono or bimaxillary) is proposed to the patient in skeletal class I or class II with high IAH, wishing a definitive solution: the forward projection of the facial and mandible releases the upper airways, increases their volume and prevents nocturnal pharyngeal collapse. Taking into account the SAHOS makes it possible to direct the patient towards an orthodontic-surgical treatment. The rise of 3D imaging (CTCT, CBCT) in recent decades makes diagnosis and analysis much more accessible. A review of the literature was conducted to evaluate the effect of different orthognathic surgeries (maxillomandibular advancement, maxillary advancement and mandibular setback, mandibular setback alone) on PAS volume, and to study the consequences on the respiratory function. 14 articles demonstrated the positive impact of MMA on the PAS volume and the improvement of OSA. In skeletal class III cases, bimaxillary surgery has an advantage over mandibular setback alone, however the authors measured a decrease in PAS volume.
