Le syndrome de sevrage du nouveau-né : étude de la symptomatologie et de la pertinence des tests diagnostiques

Abstract : Introduction: assessment tools of the Neonatal Abstinence Syndrome are 45 years old without any reassessment whereas there is a growing prevalence of the syndrome. Methods: ahe 2010–2014 data of two Hospitals in Normandy were analysed for 81newborns from drug addict mothers. Scoring, physiological parameters of new-borns and mothers were screened. New-born from Methadone substituted mothers and Buprenorphine ones were compared. Treatment protocols were analysed. The diagnosis score was to be questioned. Results: tremor (Khi 2 = 4.12), Reflex (Khi 2 = 3.86) and tone (Khi 2 = 3.6) were found more frequent in New-born from Methadone substituted mothers. In this group, Morphin Chlorhydrate is initiated at 0.36 mg/kg/J to 0.4 mg/kg/J while we found a 0.18 mg/kg/J to 0.3 mg/kg/J doses in the Buprenorphine group. There were a 3.1 +/- 0.4 scoring rate per day with a maximum of 8 +/- 1.1 scores a day. Conclusion: neonatal Abstinence Syndrome’s assessment tools are no longer relevant. They are no longer linked to nowadays priorities and hospital teams are not trained enough. Clinical differences between Methadone and Buprenorphine groups should be considered. Protocol should no longer be only clinical and may include biological screening. Preventive care and support may be the priorities. Without any feedback on long term treatments consequences, prospective studies have to be done.
