Quels sont les déterminants du taux d'occupation des Appartements de Coordination Thérapeutiques (ACT) destinés aux détenus de la maison d'arrêt de Gradignan ?

Abstract : Introduction: according to the survey of the Directorate General of Health conducted in June 2012, there is a mismatch between supply and demand of Therapeutic Coordination Apartments (ACT) for prisoners. La Case and SOS ACT Bègles are two associations working in collaboration with Gradignan prison in order to guarantee continuity of care for prison leavers who carry heavy illnesses. The activity report of the ACTs between 2013 and 2016, reveal a non-optimal occupancy rate. The main objective of our study is to make an inventory of the determinants of the occupancy rate of ACTs intended for this population. Method: it is a survey of horizontal declarative practice with the medico-social staff of the approved associations, the prison and the prisoners. All the medico-social staff has been included. Inmates were included using a linear randomization method. The data were collected using self or heterogeneous questionnaires submitted during physical, telephone or email interviews. The data was manually analyzed and transcribed into tables and charts using Microsoft Excel for Mac version 15.18. Results: the number of applications for annual admission in ACT remains low. Detainees questioned are unaware of ACTs. Social workers in the remand center have more experience with the referral circuit than doctors. The admission criteria, the knowledge and the accessibility of the two associations differ, creating an imbalance in the orientations. There is little refusal of admission, except for pathologies that do not involve medico-social care and insufficient autonomy. No social criterion leads to refusal. The administrative reasons for refusal are: an unsuitable application for the admission criteria, other possibility (s) of housing, a short period of imprisonment and the lack of available space. Conclusion and perspectives: the early identification of candidates and the dematerialization of applications via an electronic platform would facilitate orientation by the requesting structures. Strengthening exchanges between them and the associations would improve the occupancy rate of ACTs. The creation of a consensual list of admission criteria would give all candidates an equal chance. Better information of the prison population would improve their knowledge of ACTs.
