État de santé et développement cognitif des enfants dans un contexte de grande vulnérabilité maternelle : étude descriptive observationnelle d’une population girondine issue de la réunion de concertation pluridisciplinaire « Périnatalité – Grande Vulnérabilité »

Abstract : Introduction: numerous studies have objectified the negative impact of maternal medical, psychological and social vulnerabilities on the cognitive development of the child. In New Aquitaine Region, it exists a multidisciplinary meeting (MDM) “Perinatality and Extreme Vulnerability” in charge of developing a health care pathway aimed for highly vulnerable pregnant women. We wanted to assess the outcome of these women and their children. We aimed to evaluate the health and psychomotor development of the children. Material and methods: we have carried out a descriptive observational cross-sectional study. Between May 2018 and January 2019, we saw in consultation children between 18 months and 4 years old, whom mothers had been included in MDM between November 2014 and May 2017. We have collected data about mothers’ medical, psychological and social status at both date MDM and consultation times as well as informations regarding children’s health and cognitive development with valid screening tests. Results: we gathered 52 records and included 16 children. 37,5 % presented a developement considered suspicious for fine motor skills, 12,5 % had language deficiency, 43,8 % had behavioural difficulties. 43,8 % children were referred to additional cares in speech-language therapy, psychomotricity or to a Medico-psychological Centre for child and adolescent (CMPEA). Among the four dyads which psycho-social situation has stabilized, 3 children were healthy and needed no follow-up. Conclusion: the children of our research are at higher risk for cognitive and behavioral disorders. Thus, it is essential to improve the follow-up of those vulnerable women and their child and promote maternal social stability to ensure the development of the child.
