Devenir des informations préoccupantes émises par le secteur de la santé : enquête de pratique descriptive

Abstract : Childhood at risk is a public health problem: difficulty of identification, particularly vulnerable population with specific health needs, whose access to healthcare is sometimes limited. According to the law n ° 2016 -297 of March 14, 2016, child protection aims to ensure that the child’s basic need are taken into account, to support its physical, affective, intellectual and social development and to preserve its health, safety, morality and education, in the respect of its rights. This law provides, among other things, the improvement of the identification of children at risk by physicians, as well as the harmonization of practices to evaluate dangerous situations. A consensus approach of 2017 defines the "universal basic needs of the child", the "specific needs" of the child who’s been neglected or abused, and the "special needs". Child development and health are essential in these approaches. In order to understand the importance given to health in the identification and evaluation, we’ve described the modalities of assessment of the dangerous situations as well as the place of the maternal and child protection centers in these evaluations, following an investigation in child protection emitted by health professionals, on a specific territory of the department (Family Childhood Center of Aix en Provence) during 2017 : a total of 30 situations involving 43 minors. This population of minors presents in 90% of cases risk factors of vulnerability, a family medical context in 80% of situations, as well as previous child protection measures for 53% of families. Our study reveals territorial disparities in terms of assessment of dangerous situations, deviations from legislative texts, as well as the lack of a common reference system. The use of maternal and child protection centers is effective in 80% of cases for children under 6 years of age, but it isn’t always sought in case of a medical problem. In case of a medical problem, the evaluators use the practitioners in charge of the child’s follow-up, demonstrating an openness to multidisciplinary. Our study, although carried out on a reduced staff, confirms the interest of the institutional project of the Department of the Bouches du Rhône to set up the national reference system of child protection (CREAI evaluation tool), in order to harmonize the practices and to place the child’s basic needs in the process of assessment and child protection. It could also help to recreate links between health professionals and those of child protection.
